The Persistence of Extramedullary Leukemic Infiltrates during Bone Marrow Remission of Acute Leukemia.

چکیده
By BOYD A. NIES, GERALD P. BODEY, Louis B. THOMAS, GEORGE BRECHER AND E fIL J. FREIREICH F OLLOWING the successful treatment of a patient with acute leukemia, evidence of residual leukemia by bone marrow, peripheral blood, or physical examination is often absent. Invariably, however, infiltration of the bone marrow by leukemic cells recurs. Whether a few undetectable leukemic cells remain during remission or whether leukemic cells are reinduced by a persisting etiobogic agent is not known. Skipper has shown in the L1210 mouse leukemia model system both that a single leukemic cell will eventually cause death, and also that “cure” can be accomplished only in those animals in which there is a statistical possibility of having eliminated all leukemic cells.1 These observations suggest that, in mice, recurrences are due to survival of a few leukemic cells. In order to explore this problem in man, patients with acute leukemia dying during bone marrow remission were studied to determine if leukemic cell infiltrates were present in their organs and tissues.

Leukemic Infiltration of the Appendix as an Unusal Site of Extramedulary Relapse: Report of Two Cases and Review of the Literature
The appendix is an unusual site for extramedulary relapse in acute leukemia. The present case report describes two cases of late course acute lymphoblastic leukemia presented with leukemic infiltration of the appendix and complete remission of bone marrow. The signs and symptoms of the cases suggest that leukemic involvement of the appendix should be considered in the differential diagnosis of ...
متن کاملRelapse of Biphenotypic Acute Leukemia as a Breast Mass
In acute leukemia, leukemic infiltration of the breast is extremely rare. We report a case of biphenotypic acute leukemia (BAL) that presented as a breast mass. A 30-year-old woman presented with a 4-month history of a right breast mass with nipple discharge and easy fatigue. She had received chemotherapy and peripheral blood stem cell transplantation for BAL and had been in complete remission ...
متن کاملCase of pelvic relapse in a child suffering from acute lymphoblastic leukemia.
We describe here a case of an eight years old child suffering from acute lymphoblastic leukemia. She developed pelvic infiltration of leukemic cells while in bone marrow remission and receiving maintenance chemotherapy. She also developed leukemic infiltration of Central Nervous System and died of complications resulting from massive pelvic relapse. With greater number of children in bone marro...
متن کاملUnilateral Ptosis as Initial Manifestation of Acute Myeloid Leukemia
Abstract Background In Acute Myeloid Leukemia (AML), malignant clones of immature myeloid cells (primarily blasts) proliferate, replace bone marrow, circulate in blood and invade other tissues. The unique presentation of unilateral ptosis and unilateral eyelid swelling in AML has been reported during the present study. Case Report A 7-year-old girl was encountered with progressively increa...
متن کاملFace Bones Involvement and Relapse in a Case of Childhood Acute Leukemia
Midface bones are an unusual site for primary presentation and relapse in acute lymphoblastic leukemia. Herein, we describe a case of acute pre B cell lymphoblastic leukemia with leukemic infiltration of maxilla and bone marrow involvement. At the time of relapse, the patient presented again with maxilla involvement and the phenotype changed to biphenotypic lymphoblastic leukemia. Our case sug...
